Disease Management Program (DM) – The DM program has three components: (1) Education, (2) Recording & Monitoring Health Data, and (3) Messaging. Patients are enrolled in an automated system that sends reminders about important follow-up care, and notifies their physician that intervention is needed when any of several values exceed a pre-determined care threshold. The DM program is currently designed for patients who have any of five designated chronic diseases: Diabetes, Asthma, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), and Coronary Artery Disease, as well as two or more of the following risk factors: obesity, hypertension, high cholesterol, improper diet, physical inactivity, or smoking. The goal of the DM program is to address quality of care, healthcare access, appropriate utilization, coordination of care, and quality of service issues to assure that care delivery is maintained at the optimally achievable level of qualify, safety, timeless, and value.
Education – GMFT understands the importance of education to healthcare. Education includes professional development of the physician and office staff. GMFT seeks to ensure continuing education opportunities for providers of healthcare in North Texas.
Technology and Research – GMFT seeks to develop emerging technologies and assist in bringing ideas to fruition for the benefit of the community. A current interest is the development of a program for digital imaging in diabetic retinopathy, with the objective to improve overall care of diabetic patients in our community. Diabetic retinopathy is a major cause of blindness, and unfortunately only about one-third of diabetic patients regularly obtain recommended annual dilated eye exams. The identification of glaucoma and suggestion of the presence of cataracts also are detected by this technology. Technology employed in this project is the only “gold standard” program commercially available, that follows Early Treatment Diabetic Retinopathy Study protocol, and is ophthalmologist approved.
Electronic Health Records – In 2004 the United States established an Office of the National Coordinator for Health Information Technology. Its purpose is to advance development and inplementation of nationwide interoperable health information technology infrastructure to improve the quality and efficiency of health care. Genesis Medical Foundation of Texas intends to be a leader in this effort. The objectives of Electronic Health Records (EHR’s) used interactively at the point of care delivery are to be interoperable with all relevant data sources, Provided the caregiver with a body of knowledge and clinical tools, and be accessible to patients and other providers.
