Physicians interested in participating in the managed care contracts offered by
Genesis Physicians Group (GPG) must first become members of the Independent Practice
Association (IPA) and second complete all requirements for credentialing. To complete
the process for both, please refer to the appropriate Check List below addressing
required elements.

Please use the links below as needed and if you have already been working with a
Physician Services Representative (PSR) include their name in the space at the top
right corner of the membership page:
* Allied Health Professionals do not need to complete a membership application.

The estimated turnaround timeframe to present the physicians file to the appropriate
committees, is 35 days from receiving a completed application. The approximate time
frame for the physicians/AHP to received effective dates with payors is 60 days
after submission of the application by Genesis Physicians Group.
