Genesis Wellness Institute for Health (GWIH) is a joint venture between Genesis
and Best Practices Disease Management that began in 2009. The company contracts
with Medicare Advantage Plans and has Nurse Practitioners located in DFW, San Antonio,
Houston and El Paso. GWIH has the following services available:
1. Health Risk Assessments – Nurse Practitioners complete a health risk assessment
during a home visit on behalf of the health plan to determine patient risk factors.
This assessment determines how much a health plan will be paid for each member by
Medicare. The assessments also increase patient compliance and thus improves outcomes.
Genesis members benefit through our exclusive pay-for-performance program, which
includes multiple Medicare Advantage contracts that pay bonuses to physicians based
on profitability. GWIH health risk assessments ensures that revenue is maximized
by health plan and can increase physician bonuses for each product.
2. Medical records Review – GWIH will conduct Medical Record Reviews on behalf of
the health plan to help secure data necessary to calculate HEDIS scores as well
as other clinical data to analyze specific clinical outcomes.
3. Chronic Illness Patient Management – As an extension of the primary care physicians,
nurse practitioners will conduct in home visits to support the management of patients
with a chronic illness such as diabetes, congestive heart failure, chronic obstructive
pulmonary disease, asthma, hypertension and obesity. Extensive review of systems
and medication reconciliation along with educational instruction on nutrition, lifestyle
and exercises results in reduced hospital admissions and better overall health of
the patient.
4. Care Transitions Program – Nurse Practitioners will visit a discharged patient
within 24 hours and perform medicine reconciliation, review post discharge instructions
and assist in coordinating access to care, if required.
5. Mobile Diabetic Retinopathy Scans – Technicians using a state-of-the-art, seven
field of view scanners are able to detect any pathology within the eyes of the diabetic
patient. If needed, patients are referred to a retinal specialist or ophthalmologist
for care. In addition, this service assists in improving the eye exam HEDIS score
which often times is tied to a bonus provision in the Medicare Advantage contract.
